Dear colleagues:

It’s already time to fill out the information gathering questionnaire the Ordre has been sending you every year since the fall of 2008.

Deadline: December 4, 2009
All Ordre members, regardless of the member category to which they belong, must fill in the questionnaire, which they will find on the Ordre’s website in their “CA File,” by December 4, 2009. If you fail to do so, you will not receive a notice of annual fees in February and will not be entered on the Roll of the Ordre as at April 1 of next year. Only those members who have returned their duly completed questionnaire within the prescribed deadline will receive their notice of annual fees in February. They will then have until April 1, 2010 to send their payment to the Ordre.

Changes to note
As announced in the September 29 and October 6, 2009 issues of Inf@CA, changes have been made to the 2010-2011 annual fees. If you have not already done so, please read the lead article on the 2010-2011 annual fees in each of these issues.
 
Lastly, I invite you to take a few minutes right now to fill in the questionnaire online. You will find below detailed explanations of the eight steps necessary to complete the process.

To complete the information gathering questionnaire in 8 steps

1. Go to the Ordre's website at www.ocaq.qc.ca.

2. Identify yourself as a member in the appropriate box.

3. Access your "CA File” by entering your password in the appropriate box.

4. Once this information is authenticated, the screen “Step 1: Contact information – Information gathering questionnaire” will immediately pop up.

5. Answer the questions in the six steps shown at the left of the screen.

6. Once you are in the “Declaration” section, click on “Print” at the top of the screen if you would like to keep a copy of the questionnaire for your files.

7. Lastly, you must click on “Attest” to confirm the truthfulness and accuracy of this information.”

8. You will then be automatically directed to the “General Information” section of your “CA File.” If your personal or professional address is no longer valid, please make the necessary changes by clicking on “Modify” at the bottom of the screen.


Note: Members who answer yes to one of the questions in section 4 dealing with trust accounting will first be directed to an accountability report, and then redirected to the “General Information” section indicated in step 8.
