Draw for a 12-bottle case of wine

How would you like to stock your wine cellar with 12 bottles of fine wine valued at approximately $5,000? All it takes is a bit of luck - and your participation in the draw organized by the Foundation of Quebec Chartered Accountants.

Date and time of the draw: Friday, December 10, 2010, at 2 p.m.

Cost: $50

Maximum number of tickets sold: 500

Deadline for claiming your prize: December 31, 2011 at 4 p.m.

Contest rules:

1. To participate in the draw for a 12-bottle case of wine valued at approximately $5,000, participants must fill out the online entry form and make their payment by credit card through PayPal or by cheque. If the payment is made by cheque, the entry form must be printed and returned by mail, along with the cheque, to the Foundation of Quebec Chartered Accountants (the “Foundation”) at 680 Sherbrooke Street West, 18th floor, Montreal, Quebec, H3A 2S3.
2. To be valid, all entry forms must be received on line or by mail no later than December 3, 2010 at 4 p.m.
3. The contest is open to all Quebec residents aged 18 and over who purchase one or more tickets for the draw for a 12-bottle case of wine valued at approximately $5,000 to benefit the Foundation.
4. Upon receiving the entry form, the Foundation will send the participant a numbered ticket. The Foundation will keep a part of the numbered ticket bearing the participant’s contact information.
5. The entry forms and the detachable part of the numbered tickets become the property of the Foundation and will not be returned to participants. The Foundation agrees to dispose of them using all necessary precautions once the contest has ended.
6. Personal information, such as the participant’s name, mailing address, telephone number and e-mail address, is collected solely for the purpose of administering this contest and will not be used for any other purpose without consent. By providing this information, participants agree to it being used for the stated purposes.
7. The winner must agree to having his or her name and picture used for publicity purposes without compensation.
8. The chances of winning a prize are directly related to the number of entries. A maximum of 500 tickets will be sold.
9. At approximately 2 p.m. on December 10, 2010, a random draw will take place at the Ordre des comptables agréés du Québec from among all entries received.
10. The winner will be contacted by telephone within seven (7) days of the draw. The prize must be claimed no later than December 31, 2011 at 4 p.m.
11. The Foundation will choose the 12 bottles of wine.
12. The prize must be accepted as is and may not be exchanged for cash, sold or transferred. No substitutions will be allowed.
13. Refusal of the prize releases the Foundation from any and all responsibilities and obligations towards the winner.
14. Participation in this contest constitutes consent with these rules. The Foundation will be responsible for their enforcement and all decisions are final.
